Comparing four age groups (younger than 55 years, 55 to 64 years, 65 to 74 years, and 75 years and older), the researchers found that the oldest patients experienced significant improvements in knee pain, knee function, and quality of life after TKA. The youngest patients (younger than 55 years) had more pain after TKA compared with the other three age groups. The improvement achieved in pain in the age group younger than 55 years was equal to the other groups, but because they had more pain preoperatively, the final mean pain scores were lower (more pain) than the other cohorts one year after TKA. Those aged younger than 55 years were more likely to be obese before surgery (body mass index &gt;35 kg/m2), more likely to be current smokers, and more likely to have significant pain in their lumbar spine and other joints in their lower extremities than the other three age groups.

The simple score was developed using logistic regression, while the least absolute shrinkage and selection operator Cox proportional hazards regression was used to develop the comprehensive score.

A total of 119 high-risk participants and 336 low-risk participants were identified with the comprehensive score, with a false-positive and false-negative rate of 29 and 19 percent, respectively. Of the high-risk participants, 40 and 71 percent developed inflammatory arthritis within one and five years, respectively.

Changes in outcomes were assessed during 54 to 64 weeks.

From baseline to the end of week 64, the mean score increases were 22.87, 20.78, 16.60, and 24.87 on the Pain, Symptoms, Physical Function, and QOL subscales, respectively, for patients with knee osteoarthritis. The corresponding mean score increases were 22.81, 19.93, 18.77, and 22.71, respectively, for patients with hip osteoarthritis. At the end of the observation period, there was a reduction in the number of patients using any nonsteroidal anti-inflammatory drugs (43.1 to 13.5 percent).

Patients were randomly allocated to GC (1,500 mg of glucosamine + 1,200 mg of chondroitin sulfate + 500 mg of saccharum lactis; 49 patients), GCM (1,500 mg of glucosamine + 1,200 mg of chondroitin sulfate + 500 mg of MSM; 50 patients), or placebo (48 patients), administered once daily for three months. Visual analog scale (VAS) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores were measured before and at four, eight, and 12 weeks after treatment.

Three cornerstones are crucial for management of DSPN: lifestyle modification, including optimal diabetes treatment aimed at near-normoglycemia and multifactorial cardiovascular risk intervention; pathogenetically oriented pharmacotherapy (e.g., alpha-lipolic acid and benfotiamine); and treatment of the symptoms of neuropathic pain, such as analgesic pharmacotherapy and nonpharmacological options. Pain management should consider the individual risk profile and allow for improvement in quality of sleep, functionality, and general quality of life, as well as pain relief.

The analysis included eight studies, with 1,277 eligible participants.

In a subgroup analysis, diacerein yielded an improved mean reduction in the visual analog scale score of −0.44 percent compared with placebo and a −0.44 percent improvement in the Western Ontario and McMaster Universities Osteoarthritis Index&nbsp;(physical function) score. Diacerein treatment had a significant residual effect in a follow-up analysis after discontinuation. Data on drug side effects showed an increased risk for diarrhea with diacerein and withdrawal events from therapy (risk ratios [95 percent confidence intervals], 1.95 [1.03 to 2.47] and 0.93 [075 to 1.15], respectively).

<p style="box-sizing: border-box; margin: 0px 0px 15px; padding: 0px; font-size: 20px; line-height: var(--arrow-lh-5); font-family: var(--arrow-typeface-secondary); font-stretch: normal; word-break: break-word; background-color: #ffffff;">Zhiyu Wang, from the South China Hospital of Shenzhen University, and colleagues examined the safety and efficacy of DIA plus GlcN-HCl in the treatment of KOA in a retrospective study involving 116 KOA patients admitted between August 2018 and August 2021. Of the patients, 55 and 61 received DIA monotherapy (control group) and DIA + GlcN-HCl (observation group), respectively.

The occurrence of complications was affected by tumor necrosis factor-α, interleukin-1β, high-sensitivity C-reactive protein, and treatment mode.

<p style="box-sizing: border-box; margin: 0px 0px 15px; padding: 0px; font-size: 20px; line-height: var(--arrow-lh-5); font-family: var(--arrow-typeface-secondary); font-stretch: normal; word-break: break-word; background-color: #ffffff;">Jordi Puigdellívol Grifell, M.D., from the Medical Staff FC Barcelona in Spain, and colleagues recruited 186 patients with knee and/or hip OA to examine the effectiveness of a nutritional supplement. The supplement contained hydrolyzed collagen (3,000 mg), chondroitin sulfate (800 mg), glucosamine sulfate (700 mg), turmeric extract (250 mg), and devil's claw (150 mg) and was taken once daily for six months.
<p style="box-sizing: border-box; margin: 0px 0px 15px; padding: 0px; font-size: 20px; line-height: var(--arrow-lh-5); font-family: var(--arrow-typeface-secondary); font-stretch: normal; word-break: break-word; background-color: #ffffff;">The researchers found that after three and six months of treatment, participants experienced a significant reduction in self-perceived pain (mean reduction ± standard deviation, 1.99 ± 1.05 and 3.57 ± 1.39, respectively). At three and six months of treatment, the Lequesne Functional Index score was significantly reduced (3.86 ± 2.94 and 6.73 ± 4.30, respectively). There was also a significant reduction in the Western Ontario and McMaster Universities Arthritis (WOMAC) Index (14.24 ± 10.04 and 26.43 ± 17.35, respectively). There were significant reductions in WOMAC subdomains. During the study, there were no reports of severe adverse events.

Participants were randomly assigned to six months of diacerein 50 mg once daily for one month and twice daily thereafter or celecoxib 200 mg once daily (187 and 193, respectively).

Similar results were seen in a supportive analysis of the intention-to-treat population. No significant between-group difference was seen in other outcomes. A low incidence of treatment-related adverse events was seen, and these incidences were balanced between the groups, apart from an increased incidence of diarrhea with diacerein (10.2 versus 3.7 percent).

In addition, it has an analgesic effect, reducing the pain associated with healing. Postulated mechanisms of action include targeting the early stage of inflammation, characterized by a steep rise in α1-antitrypsin and α2-macroglobulin, which leads to fibrinolytic shutdown and consequent maintenance of inflammation and edema following acute injury; and assisting with maintenance of α1-antitrypsin for a long duration, countering the activity of proteolytic enzymes and their degradative effects, leading to a reduction in inflammatory milieu as well as faster healing. The trypsin:chymotrypsin combination is efficacious and safe for resolving inflammation and edema and improving healing, and is used in clinical indications such as accidental, surgical, and orthopedic injuries; burn injuries; and sciatica.

Role of B12 Supplementation Examined for Neuropathic Pain

<p style="box-sizing: border-box; margin: 0px 0px 15px; padding: 0px; font-size: 20px; line-height: var(--arrow-lh-5); font-family: var(--arrow-typeface-secondary); font-stretch: normal; word-break: break-word; background-color: #ffffff;">Thitima Kasemsuk, from Mahidol University in Thailand, and colleagues examined 16-week treatment with bromelain in mild-to-moderate knee OA. Forty patients with knee OA were randomly assigned to oral bromelain (500 mg/day) or diclofenac (100 mg/day). The primary outcome was the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). The short-form 36 (SF-36) was measured as the secondary outcome.

Two patients receiving diclofenac had adverse events resulting in discontinuation. The treatment difference was inconclusive. Compared with baseline, at week 16 of bromelain treatment, there was improvement in total WOMAC scores (12.2 versus 25.5), pain subscales (2.4 versus 5.6), stiffness subscales (0.8 versus 2.0), and function subscales (9.1 versus 17.9) and in the physical component of the SF-36 (73.3 versus 65.4). Higher plasma malondialdehyde (MDA), nitrite, and prostaglandin E2&nbsp;(PGE2) in lipopolysaccharide (LPS)-stimulated whole blood and lower plasma α-tocopherol were seen for OA patients. At week 16 of treatment, plasma MDA and LPS-stimulated PGE2&nbsp;production were decreased.

F. Walker, from the University of Reading in the United Kingdom, and colleagues examined the effects of bromelain on mild acute knee pain of less than three months duration in otherwise healthy adults in an open, dose-ranging postal study in volunteers. Participants were randomly allocated to receive 200 mg or 400 mg bromelain per day. At baseline and after a one-month intervention with bromelain, two validated questionnaires (Western Ontario and McMaster Universities [WOMAC] knee health index and the Psychological Well-Being Index) were completed; the study was completed by 77 participants.

Compared with the low-dose group, improvements in total symptom score and the stiffness and physical function dimensions were significantly greater in the high-dose group. Overall psychological well-being was significantly improved in both groups after treatment versus at baseline; a significant dose-response relationship was seen.

Balance exercises, yoga, cognitive behavioral therapy, acupuncture, thermal modalities, kinesiotaping for first CMC OA, orthoses for hand joints other than the first CMC joint, patellofemoral bracing for patellofemoral knee OA, radiofrequency ablation and topical capsaicin for knee OA, topical NSAIDs, intra-articular steroid injections and chondroitin sulfate for hand OA, and acetaminophen, duloxetine, and tramadol were conditionally recommended.

The patients were assigned to receive oral preparation of trypsin:chymotrypsin or placebo (25 in each group).

Recovery time was shorter in the chymotrypsin and trypsin group; recovery time was less than one week in 80 and 68 percent of patients in the chymotrypsin and trypsin and placebo groups, respectively.

Zaid, from the First Affiliated Hospital of Xiamen University in China, and colleagues screened patients undergoing ATFL repair for depression preoperatively with the Patient Health Questionnaire. The study included 120 patients who were identified as being possibly depressed. Patients were randomly assigned to the Duloxetine group (one day preoperatively and six weeks postoperatively) or to placebo (60 patients to each group). At 12, 24, 48, and 72 hours postoperatively and at follow-up visits one, three, and six months after surgery, painkillers and opioid consumption, pain scores, and patient satisfaction were recorded.

Compared with women without diabetes, those with T2D had 9.7 percent lower grip strength, 9.9 percent slower gait speed, and 13.9 percent slower timed up-and-go time. There were 1,071 incident fractures, 853 major osteoporotic fractures, and 232 hip fractures during a median follow-up of 7.3 years. T2D was associated with an increased risk for any fracture and major osteoporotic fractures in adjusted Cox regression models (hazard ratios [95 percent confidence intervals], 1.26 [1.04 to 1.54] and 1.25 [1.00 to 1.56], respectively).

Mackenzie, M.B.Ch.B., from the University of Dundee in the United Kingdom, and colleagues conducted a prospective, randomized, noninferiority trial of Febuxostat versus allopurinol in patients aged 60 years or older with gout. After a lead-in phase during which the allopurinol dose was optimized toward achieving serum urate &lt;6 mg/dL, patients were randomly assigned to allopurinol or to start Febuxostat at 80 mg/day, increasing to 120 mg/day if necessary.

The researchers found that for the incidence of the primary end point (composite of hospitalization for nonfatal myocardial infarction or biomarker-positive acute coronary syndrome; nonfatal stroke; or cardiovascular death), Febuxostat was noninferior to allopurinol (1.72 versus 2.05 events per 100 patient-years; adjusted hazard ratio, 0.85 [95 percent confidence interval, 0.70 to 1.03]; P &lt; 0.0001). Overall, 7.2 percent of patients in the Febuxostat group died and 57.3 percent in the safety analysis set had at least one serious adverse event. In the allopurinol group, 8.6 percent of patients died and 59.4 percent had one or more serious adverse events.

<p style="box-sizing: border-box; margin: 0px 0px 15px; padding: 0px; font-size: 20px; line-height: var(--arrow-lh-5); font-family: var(--arrow-typeface-secondary); font-stretch: normal; word-break: break-word; background-color: #ffffff;">Vladimir Skljarevski, M.D., from Lilly Research Laboratories in Indianapolis, and colleagues examined the efficacy and safety of Duloxetine for treatment of CLBP in a randomized, placebo-controlled trial. Adults without depression and with non-neuropathic CLBP were randomly assigned to receive Duloxetine or placebo for 13 weeks. During the first seven weeks, the dose of Duloxetine was 60 mg.

Significant improvements were also seen on the Patient Global Impression of Improvement; Roland-Morris Disability Questionnaire-24; BPI-Severity and average BPI-Interference; and weekly mean of the 24-hour average pain, night pain, and worst pain in the Duloxetine group. Compared with the placebo group, significantly more patients in the Duloxetine group discontinued due to adverse events (13.9 versus 5.8 percent). In the Duloxetine group, the most common treatment-emergent adverse events included nausea, dry mouth, fatigue, hyperhidrosis, dizziness, and constipation.

The PIONEER PLUS trial compared doses of oral GLP-1 higher than currently approved (25 and 50 mg daily versus currently approved 14 mg daily), demonstrating significantly superior HbA1c reduction and body weight reduction at both of the higher doses compared with 14 mg in adults with type 2 diabetes. The overall safety of oral GLP-1 at these higher doses was consistent with it’s safety profile, with the most frequent adverse events reported being gastrointestinal in nature (i.e., nausea, vomiting).

Ten randomized controlled trials (1,242 participants) were included and reviewed.

ALA treatment did not produce favorable results for the visual analog scale score, neuropathy impaired score-lower limb, or nerve conduction study results.

"Nutritional supplementation for diabetic complications may be a preventive strategy in diabetic care."
