Composition

Orlistat  120 mg

Properties

Orly capsule contains the lipase inhibitor Orlistat used for the management of obesity in conjunction with a reduced-calorie diet. Orlistat reduces the absorption of dietary fat through inactivation of gastric and pancreatic lipases in the stomach and small intestine, consequently, the dietary fat remains in the form of undigested triglycerides, and is not further hydrolyzed into absorbable fatty acids and monoglycerides, these results in lowering of calorie production and leads to an effective weight control.

Indications

  • For management of weight loss and weight control for adults and adolescents.
  • For reduction of weight regain liability post a weight loss attainment.

Contra-indication

Chronic malabsorption syndrome, cholestasis and breast-feeding.

Side effects

  • Oily discharge from rectum, flatulence, faecal urgency, liquid or oily stools, faecal incontinence, abdominal distension and pain (gastro-intestinal effects last for few weeks and are minimized by reduced fat intake).
  • Less frequent side effects which appear only after long period of therapy are tooth and gingival disorders; respiratory infections; anxiety, headache; menstrual disturbances, urinary-tract infection.

Dosage

  • Adult dose: one 120 mg capsule to be taken immediately before each principal meal (up to a maximum of 360 mg daily); It may be taken also during meals, or up to 1 hour after meals, the dose is omitted if missed or the meal is fat-free.
  • Treatment may extend for a period longer than 12 weeks when a 5% weight loss is achieved during this period as measured from initial therapy.
  • Orly is not recommended for children

Drug interactions

  • Orlistat possibly reduces plasma concentration of amiodarone and absorption of cyclosporin.
  • Avoid concomitant use with acarbose .
  • Monitoring  of anticoagulant  effect of coumarins should be done if used with orlistat.
  • Orlistat may impair absorption of fat-soluble vitamins.

Precaution

  • Organic causes of obesity (such as hypothyroidism) should be excluded.
  • Gastrointestinal related symptoms may increase when Orly is taken with a fatty meal.
  • No sufficient studies are available for pregnancy so it should not be used during pregnancy or by nursing women.
  • Orly capsule is prescribed with caution to patients with a history of hyperoxaluria or calcium oxalate nephrolithiasis.

Storage

  • Store at a temperature not exceeding 30°C.
  • Keep out of reach of children.
